Nanosilica coatings are considered a simple physical treatment to alleviate the effect of cohesion on powder flowability. In limestone powders, these coatings buffer the rise in cohesion at high temperatures. Here, we investigate the role of particle size in the efficiency (and resilience) of these layers. To this end, this work examines a series of four limestone powders with very sharp particle size distributions: average particle size ranged from 15 to 60 µm. All the samples were treated with nanosilica at different concentrations from 0 to 0.82 wt %. Powders were subjected to short- and long-term storage conditions in calcium looping based systems: temperatures that vary from 25 to 500 °C and moderate consolidations (up to 2 kPa). Experiments monitored powder cohesion and its ability to flow by tracking the tensile strength of different samples while fluidized freely. Fluidization profiles were also used to infer variation in packings and the internal friction of the powder bed. Interestingly, for particle sizes below 50 µm, the nanosilica treatment mitigated cohesion significantly-the more nanosilica content, the better the flowability performance. However, at high temperatures, the efficiency of nanosilica coatings declined in 60 µm samples. Scanning electron microscopy images confirmed that only 60 µm samples presented surfaces barely coated after the experiments. In conclusion, nanosilica coatings on limestone are not stable beyond the 50 µm threshold. This is a critical finding for thermochemical systems based on the calcium looping process, since larger particles can still exhibit a significant degree of cohesion at high temperatures.

Intertidal ecosystems are key habitats that are being replaced by artificial hard substrates due to the increment of human activities in coastal areas. These new substrates host generally less biodiversity mainly due to differences in complexity and composition. This is a global phenomenon and has led to the development of strategies in the framework of eco-engineering. However, mitigating measures, such as new eco-designs, must cope with the high spatial variability of the region where they are applied. Therefore, in order to assess if differences in biodiversity detected at local scales in previous studies could be scaled up to predict patterns at a wider scale, we studied taxa richness and taxonomic structure of intertidal communities across the Alboran Sea (western Mediterranean Sea). We compared four different types of artificial substrates (cubes, rip-raps, seawalls and tetrapods) to assess which produces less impact. Overall, artificial substrates host low benthic biodiversity, specially on seawalls, whereas boulder-like artificial structures such as rip-raps were more similar to natural ones. Nevertheless, the effect of a particular type of artificial structure at a regional scale seems unpredictable, highlighting the challenge that eco-engineering measures face in order to establish global protocols for biodiversity enhancement and the importance of local scale in management programmes.

ABSTRACT This article proposes two methodologies for the detection of lesions in the retina, which may indicate the presence of diabetic retinopathy (DR). Through the use of digital image processing techniques, it is possible to isolate the pixels that correspond to a lesion of RD, to achieve segmenting microaneurysms, the edges of the objects contained in the image are highlighted in order to detect the contours of the objects to select by size those that meet an area of 15 to 25 pixels in the case of 512x512 images and identify the objects as possible microaneurysms, while for the detection of exudates the green channel is selected to contrast the luminous objects in the retinography and from the conversion to gray scale, a histogram is graphed to identify the ideal threshold for the segmentation of the pixels that belong to the exudates at the end of the optical disk previously identified by a specialist. A confusion matrix supervised by an ophthalmologist was created to quantify the results obtained by the two methodologies, obtaining a specificity of 0.94 and a sensitivity of 0.97, values that are outstanding to proceed with the classification stage.


RESUMEN Este artículo propone dos metodologías para la detección de lesiones en la retina, que pueden significar la presencia de retinopatía diabética (RD). Mediante el uso de técnicas de procesamiento de imágenes digitales se logra aislar los pixeles que corresponden a una lesión propia de RD, para lograr segmentar microaneurismas se resaltan los bordes de los objetos contenido en la imagen con la finalidad de detectar los contornos de los objetos para seleccionar por tamaño los que cumplan con un área de 15 a 25 pixeles en el caso de imágenes de 512x512 y se identifiquen los objetos como posibles microaneurismas, mientras que para la detección de exudados se selecciona el canal verde para contrastar los objetos luminosos en la retinografía y a partir de la conversión a escala de grises se grafica un histograma para identificar el umbral idóneo para la segmentación de los pixeles que pertenecen a los exudados al final eliminar el disco óptico previamente identificado por un especialista. Se creó una matriz de confusión supervisada por un oftalmólogo para cuantificar los resultados obtenidos por las dos metodologías obteniendo una especificidad del 0.94 y una sensibilidad del 0.97, unos valores que son sobresalientes para proceder con la etapa de clasificación.

The present work aims to establish a guide to action, agreed by anaesthesiologists and neurophysiologists alike, to perform effective intraoperative neurophysiological monitoring for procedures presenting a risk of functional neurological injury, and neurosurgical procedures. The first section discusses the main techniques currently used for intraoperative neurophysiological monitoring. The second exposes the anaesthetic and non-anaesthetic factors that are likely to affect the electrical records of the nervous system structures. This section is followed by an analysis detailing the adverse effects associated with the most common techniques and their use. Finally, the last section describes a series of guidelines to be followed upon the various intraoperative clinical events.

Resumen Introducción: La obesidad es uno de los problemas de salud pública más graves del siglo XXI. Durante la etapa infantil la madre es el cuidador principal y modelador de las conductas de salud del hijo; factores como metas maternas y prácticas de alimentación determinan el índice de masa corporal en el hijo. Objetivo: Identificar si las metas y prácticas maternas de alimentación, así como el consumo de alimentos predicen el índice de masa corporal en niños preescolares. Método: Estudio descriptivo correlacional. Participaron 217 diadas (madre e hijo preescolar). Se aplicó el Cuestionario Elección de Alimentos, Cuestionario Integral de Prácticas de Alimentación y el Cuestionario de Frecuencia de Alimentos. Se midió peso y talla de las diadas, se calculó el índice de masa corporal y se obtuvo el estado nutricio. Se aplicó estadística descriptiva e inferencial a través de regresión lineal múltiple. Resultados: La meta conveniencia, el consumo de alimentos lácteos y cereales dulces, edad y el índice de masa corporal materno, contribuyeron al índice de masa corporal en el hijo preescolar, varianza explicada de 28.3%. Conclusión: Las variables estudiadas tuvieron mínima contribución al índice de masa corporal del hijo. Se recomienda realizar estudios multivariados para explicar de forma más integral el exceso de peso infantil.


Abstract Introduction: Obesity is one of the most severe public health problems of the 21st century. During childhood, the mother is the main care provider and model of the health the son´s behaviors; and thus, factors such as the mother's objectives and the food choices habits have an impact on the body mass index of the son. Objective: To identify if the objectives and food choices of the mother, as well as the specific food can predict the body mass index in pre-school children. Method: This is a descriptive and correlational study. 217 dyads (mother and pre-school son) participated. The Food Choices Questionnaire, the Integral Questionnaire on Feeding Practices and the Food Frequency Questionnaire were administered. Weight and size were measured. The body mass index was calculated and nutritional status was estimated. Descriptive and inferential statistics, including multiple linear regression, were obtained. Results: Convenience as an objective, the consumption of dairy products and sweet cereals, the age, and the mother's body mass index, had an impact on the body mass index of the son and accounted for 28.3% of the explained variance. Conclusion: The studied variables had a minimal contribution to the BMI of the sons. It is recommended to carry out multivariate studies in order to explain more integrally the excess of weight among pre-school children.

The epidemiology of non-tuberculous mycobacteria (NTM) in Spain is largely unknown because systematic reporting is not compulsory. The aim of our study was to describe the frequency and diversity of NTM species in our region and their distribution according to the source sample, gender, and age of the patients. We performed a multicenter study of all NTM isolated in 24 public hospitals in Madrid from 2013 to 2017. A total of 6.923 mycobacteria were isolated: 4535 (65.5%) NTM, and 2.388 (34.5%) Mycobacterium tuberculosis complex (MTB). Overall, 61 different NTM species were identified. The most frequently isolated species were Mycobacterium avium complex (47.7%), M. lentiflavum (12.2%), M. gordonae (9.2%), M. fortuitum (8.9%), and M. abscessus (3.9%). Whereas MTB cases were stable during the study period, the number of NTM isolates increased considerably from 930 isolates in 2013 to 1012 in 2017; a sharp increase occurred in the last year. The rise in NTM isolates was mostly due to M. lentiflavum, M. kansasii, and M. abscessus mainly isolated from respiratory specimens in patients older than 60. The increase in isolation rate of NTM in our region is consistent with the increasing rates reported worldwide in the last decades. The rise in NTM isolates was mainly attributed to M. lentiflavum but it also should be noted the increasing of species with high pathogenic potential such as M. kansasii and M. abscessus.

Some bacterial peptidyl-prolyl cis/trans isomerases (PPIases) are involved in secretory protein folding after the translocation step. Streptomyces lividans has been used as a host for engineering extracellular overproduction of homologous and heterologous proteins in industrial applications. Although the mechanisms governing the major secretory pathway (Sec route) and the minor secretory pathway (Tat route) are reasonably well described, the function of proteins responsible for the extracellular secretory protein folding is not characterized as yet. We have characterized a Tat-dependent S. lividans FK506-binding protein-like lipoprotein (FKBP) that has PPIase activity. A mutant in the sli-fkbp gene induces a secretion stress response and affects secretion and activity of the Sec-dependent protein α-amylase. Additionally, propagation in high copy number of the sli-fkbp gene has a positive effect on the activity of both the overproduced α-amylase and the overproduced Tat-dependent agarase, both containing proline cis isomers. Targeted proteomic analyses showed that a relevant group of secreted proteins in S. lividans TK21 are affected by Sli-FKBP, revealing a wide substrate range. The results obtained indicate that, regardless of the secretory route used by proteins in S. lividans, adjusting the expression of sli-fkbp may facilitate folding of dependent proteins when engineering Streptomyces strains for the overproduction of homologous or heterologous secretory proteins.

Biomasses valorization by pyrolysis is a good option for reducing environmental problems. In this study, the environmental performance of three Mexican biomass valorizations (castor husk, coffee pulp and Pinus sawdust) by the pyrolysis was compared. The environmental impacts of all equipment involved in pyrolysis were evaluated. In addition, the financial viability of pyrolysis technology of coffee pulp was studied. The biomass with the lowest impact for all the selected categories was the Pinus sawdust, followed by castor husk and coffee pulp. The GWP category had values greater than 700 kg CO2eq for all the biomass studied. GWP category is caused by the emissions, mainly due to the high amounts of CH4 and CO2 released for all the studied biomasses. Furthermore, the equipment with the greatest impact are the separator, the pyrolyzer and the cyclone. Finally, it was observed that even the least favorable biomass with the environment is viable from a financial point of view.

RESUMEN: La paquidermoperiostosis es un raro desorden hereditario caracterizado por periostosis, paquidermia y acropaquia. Su expresividad es variable, por lo que son infrecuentes las formas completas de este síndrome. Se postula que sus manifestaciones clínicas se debana la formaciónexcesiva de colágenoyla desregulacióndeproteínas de la matrizdebido a lahiperactivaciónfibroblástica. Reportamos el caso de un varón de 32 años, quien desde los 18 años presenta edema en extremidades asociado a alteraciones en cara y cuero cabelludo.Presentamos el caso por haberse manifestado deforma completa, que es infrecuente, y por su semejanza con otras enfermedades.


ABSTRACT: Pachydermoperiostosis or primary hypertrophic osteoarthropathy, also known as Touraine-Solente-Golé syndrome, is a rare disorder, frequently inherited, characterized by periostitis of long bones,pachydermia (thickening of the skin) andacropachia. Expression tends to be variable, and so complete versions of the syndrome are infrequent. Abnormalities in fibroblast functionality have been implicated, along with an increase in collagen fibers´ synthesis.We report the case of 32-year-old man that consulted for cutaneous manifestations (thickening of face, scalp, hands and feet´s skin) since 18 years-old. We highlight the importance of the complete form of presentation and due to its similarity to other diseases.

RESUMO

Paper wasps (subfamily Polistinae) are one of the four main groups of eusocial insects in the Neotropics. They are medically important for the frequent stings inflicted on humans, but at the same time are valuable predators of pest insects. Nonetheless, there are no updated keys for the identification of the Central American species. Here we provide keys to the 18 genera and 106 species known to occur in Costa Rica, illustrated with one hundred original line drawings.


Las avispas de la subfamilia Polistinae son uno de los cuatro grupos principales de insectos eusociales en el neotrópico, y son de importancia económica tanto por sus picaduras como por su papel en control biológico. Sin embargo, no existen claves actualizadas para la identificación de las especies de América Central. Aquí se proveen claves ilustradas para los 18 géneros y las 106 especies conocidas de Costa Rica y se incluyen cien dibujos originales.

Sexual ornaments contribute substantially to phenotypic diversity and it is particularly relevant to understand their evolution. Ornaments can assume the function of signals-of-quality that the choosy sex uses to evaluate potential mating partners. Often there are no obvious direct benefits and investment into mate choice is primarily rewarded by beneficial alleles that are inherited to the offspring. Inter-sexual communication via sexual ornaments requires honesty of the sexual signal, yet the question of what maintains honesty remains only partially solved. One solution is that honesty is maintained by trait expression being dependent on individual condition, since condition-dependent trait expression offers an effectively inexhaustible source of genetic variability. Here we test in the highly sexually dimorphic club-legged grasshopper Gomphocerus sibiricus if putative sexual ornaments, in particular the striking front-leg clubs, are more strongly affected by a lipopolysaccharide (LPS) immune challenge than putatively not sexually selected traits. Our results show overall little condition-dependent expression of morphological and song traits, with sexually selected traits exhibiting effects comparable to nonsexually selected traits (with the possible exception of stridulatory file length and syllable-to-pause ratio in advertisement songs). Interestingly, field observations of individuals of lethally parasitized individuals suggest that a very strong environmental challenge can specifically affect the expression of the front-leg clubs. The presence of 1% of males in natural populations with missing or heavily deformed clubs plus 5% with minor club deformations furthermore indicate that there are risks associated with club development during final ecdysis and this might act as a filter against deleterious alleles.

Particulate wastes drive benthic organic enrichment from cage fish farming. Differentiation between faeces and uneaten feed estimates at cage level are of great value to both economize the feeding process and reduce waste. This study estimates the particulate waste outflowing cages at different depths and orientations, and the wasted feed component by combining in situ measurements and modelling. Particulate matter flux (PMF) was greater vertically through the cage bottoms (60.89%), but lateral outflow was also substantial (39.11%). PMF occurs all around the cages, and the influence of the mainstream current was low. Wasted feed was greatly variable, reaching high values (about 50% of supplied feed. The self-application of feed wastage monitoring and estimates by fish farmers is recommended to improve sustainability.

INTRODUCTION: The clinical presentation of patients with soft-tissue sarcoma is highly variable. Most patients present with a painless mass, typically one that is increasing in size, and few have systemic symptoms such as fever, weight loss, or malaise. Soft tissue sarcomas can initially present as, or even be misdiagnosed as, deep venous thrombosis (DVT), leading to a late diagnosis. CASE REPORT: A 51-year-old woman presented to the hospital with complaints of pain and swelling in her left thigh, interpreted as an infected hematoma with an associated deep vein thrombosis and treated accordingly. The patient presented to our emergency department two more times. In the last visit and due to an unresolving clinical scenario a MRI and surgical byopsies were made that confirmed a sarcoma diagnosis. DISCUSSION: When a patient presents with an expanding, nontraumatic mass simulating a haematoma, several other differential diagnoses should be considered including aneurysm, bleeding tendency, chronic expanding haematoma and soft-tissue sarcoma. The growth of the tumor undetected while being treated for the DVT and then posteriorly for the hematoma, was without a doubt dismal to the patient, so earlier diagnosis would have been preferable. CONCLUSION: When a patient presents with an unusual history of hematoma in the extremities, it is necessary to consider the possibility of a malignant soft tissue tumor.

Due to the depletion of fossil fuel reserves and the environmental issues derived from their use, biomass seems to be an excellent source of renewable energy. In this work, the kinetics of the pyrolysis and combustion of three different biomass waste samples (two dairy manure samples before (Pre) and after (Dig R) anaerobic digestion and one swine manure sample (SW)) was studied by means of thermogravimetric analysis. In this work, three iso-conversional methods (Friedman, Flynn-Wall-Ozawa (FWO) and Kissinger-Akahira-Sunose (KAS)) were compared with the Coats-Redfern method. The Ea values of devolatilization stages were in the range of 152-170kJ/mol, 148-178kJ/mol and 156-209kJ/mol for samples Pre, Dig R and SW, respectively. Concerning combustion process, char oxidation stages showed lower Ea values than that obtained for the combustion devolatilization stage, being in the range of 140-175kJ/mol, 178-199kJ/mol and 122-144kJ/mol for samples Pre, Dig R and SW, respectively. These results were practically the same for samples Pre and Dig R, which means that the kinetics of the thermochemical processes were not affected by anaerobic digestion. Finally, the distributed activation energy model (DAEM) and the pseudo-multi component stage model (PMSM) were applied to predict the weight loss curves of pyrolysis and combustion. DAEM was the best model that fitted the experimental data.

BACKGROUND: Intestinal obstruction is one of the most frequent surgical emergencies. Its diagnosis is essentially based on clinical history, physical exploration and image tests. The aim of this study was to analyze the diagnostic value of acute phase reactants in patients with benign versus malign intestinal obstruction. METHOD: Historical cohort study of 53 patients who underwent surgery because of intestinal obstruction and/or non-obstructive colorectal cancer. The patients were placed in three groups: group 1 (colorectal cancer with intestinal obstruction) (n=23), group 2 (benign intestinal obstruction)(n=10) and group 3 (non-obstructive cancer of the colon)(n=20). We determined the initial plasma values of the C-reactive protein (CRP) and the lactate dehydrogenase (LDH)enzyme. RESULTS: CRP was quantitatively higher in patients with benign intestinal obstruction (group 2) (p=0.001), while LDH was quantitatively higher in group 1 (patients with obstructive cancer). The plasma levels of LDH were significantly greater in the groups with intestinal obstruction (groups 1 and 2) than in patients without obstruction (p<0.001). Plasma levels of CRP above 11 mg/l and of LDH above 317 U/L showed an acceptable diagnostic value for differentiating patients with intestinal obstruction, with areas under the ROC curve of 80% (CI 95% = 68-92%) and 86% (CI 95%= 75-96%)respectively. Their diagnostic value for differentiating benign or malign origin is lower, with areas under the ROC curve of 56% for levels of CRP > 24 ng/l (CI 95% = 30-82%) and 52% (CI 95% = 29-74%) for levels of LDH > 359 U/L. CONCLUSION: Determination of plasma concentrations of CRP can help in the diagnosis of intestinal obstruction and indicate its benign or malign origin in emergency services.

AIM: Our aim was to validate a novel use of C-reactive protein (CRP) measurement to identify postoperative infectious complications in patients undergoing colorectal surgery, and to compare the predictive value in this setting against white blood cell (WBC) count and neutrophil-to-lymphocyte ratio (NLR). METHOD: This was a retrospective study of CRP, NLR and WBC measurements in patients undergoing colorectal surgery. CRP, NLR and WBC were recorded on the second postoperative day and on the day of infectious complication (patients who developed infectious complications) or within 3 days prior to discharge (subjects with no complications). The test for detecting infectious complications consisted of comparing the value of the inflammatory marker on the day on which a complication was suspected against the value recorded on the second postoperative day. The test was considered positive if a given value was higher than the registered peak at postoperative day 2. Factors influencing the postoperative peak CRP were also studied. RESULTS: A total of 254 patients were retrospectively studied. Patients whose CRP value was higher than on the second postoperative day had a diagnostic accuracy for infectious complications of up to 94.4% and sensitivity, specificity, positive predictive value and negative predictive value of up to 97.4%, 93.4%, 85.7% and 99.1%, respectively. Poorer results were observed when WBC count and NLR were used rather than CRP measurement. Multiple linear regression analysis showed that surgical procedure and approach, as well as additional resections, were independent factors for 48 h peak CRP. CONCLUSION: C-reactive protein is a better parameter than WBC count and NLR for detecting infectious complications. Our proposed methodology presents good diagnostic accuracy and performance and could potentially be used for any surgical procedure.

RESUMO

A detailed analysis of the literature on consciousness and cognition mechanisms based on the neural networks theory is presented. The immune and inflammatory response to the anesthetic-surgical procedure induces modulation of neuronal plasticity by influencing higher cognitive functions. Anesthetic drugs can cause unconsciousness, producing a functional disruption of cortical and thalamic cortical integration complex. The external and internal perceptions are processed through an intricate network of neural connections, involving the higher nervous activity centers, especially the cerebral cortex. This requires an integrated model, formed by neural networks and their interactions with highly specialized regions, through large-scale networks, which are distributed throughout the brain collecting information flow of these perceptions. Functional and effective connectivity between large-scale networks, are essential for consciousness, unconsciousness and cognition. It is what is called the "human connectome" or map neural networks.

BACKGROUND: Colour polymorphisms are a fascinating facet of many natural populations of plants and animals, and the selective processes that maintain such variation are as relevant as the processes which promote their development. Orthoptera, the insect group that encompasses grasshoppers and bush crickets, includes a particularly large number of species that are colour polymorphic with a marked green-brown polymorphism being particularly widespread. Colour polymorphism has been associated with the need for crypsis and background matching and background-dependent homochromy has been described in a few species. However, when and how different environmental conditions influence variation in colour remains poorly understood. Here we test for effects of background colour and ambient temperature on the occurrence of colour morph switches (green to brown or brown to green) and developmental darkening in the alpine dwelling club-legged grasshopper Gomphocerus sibiricus. RESULTS: We monitored individually housed nymphae across three of their four developmental stages and into the first week after final ecdysis. Our data show an absence of colour morph switches in G. sibiricus, without a single switch observed in our sample. Furthermore, we test for an effect of temperature on colouration by manipulating radiant heat, a limiting factor in alpine habitats. Radiant heat had a significant effect on developmental darkening: individuals under low radiant heat tended to darken, while individuals under high radiant heat tended to lighten within nymphal stages. Young imagoes darkened under either condition. CONCLUSIONS: Our results indicate a plastic response to a variable temperature and indicate that melanin, a multipurpose pigment responsible for dark colouration and presumed to be costly, seems to be strategically allocated according to the current environmental conditions. Unlike other orthopterans, the species is apparently unable to switch colour morphs (green/brown) during development, suggesting that colour morphs are determined genetically (or very early during development) and that other processes have to contribute to crypsis and homochromy in this species.

INTRODUCTION: Walking Clinic is an innovative, efficient and easily reproducible concept adapted to ambulatory surgery. It consists of a preoperative single day work-up, with a surgeon, an anesthetist and a nurse. The aim of this study was to evaluate patient satisfaction and its determinants. METHODS: A survey was applied to 171 patients (101 of the Walking Clinic group and 70 not engaged in this new concept). Patient satisfaction was assessed evaluating five major questionnaire items: secretariat (quality of the information and support given), physical space (overall comfort and cleanliness), nurses and medical staff (willingness and expertise), and patients (waiting time until pre-operative consults and exams, waiting time until being scheduled for surgery, surgery day waiting time and postoperative pain control). Furthermore, overall assessment of the received treatment, and probability of patient recommending or returning to our ambulatory unit were also analyzed. RESULTS: Walking Clinic group had overall better results in the five major questionnaire items assessed, with statistical significance, except for the physical space. It also showed better results regarding the sub-items postoperative pain control, waiting time until being scheduled for surgery and surgery day waiting time. DISCUSSION: The results confirm better patient satisfaction with this new concept. CONCLUSION: The Walking Clinic concept complements all the tenets of ambulatory surgery, in a more efficient manner.

RESUMO

The valorization of three different manure samples via pyrolysis and combustion processes was evaluated. Dairy manure (sample Pre) was biologically pretreated by anaerobic digestion (sample Dig R) whereas swine manure (sample SW) was pretreated by a biodrying process. Thermal behavior of manure samples were studied by means of thermogravimetric analysis coupled with mass spectrometry (TGA-MS). These processes could be divided into four general stages: dehydration, devolatilization, char transformation (oxidation for combustion) and inorganic matter decomposition. The main differences observed among the samples were attributed to their different composition and pretreatment. The economic feasibility, energetic and environmental impacts of pyrolysis and combustion technologies for dairy samples were carried out by means of life cycle assessment (LCA) methodology. Four different scenarios were analyzed. The economic feasibility of the pyrolysis process was demonstrated, being sample Dig R the best environmental option. However, the combustion of sample Pre was the best energetic option.
